Harrodsburg, Kentucky – Ephraim McDowell James B. Haggin Hospital is now accepting patient referrals for its pharmacist-managed Anticoagulation Clinic. It will be staffed by Stacey Rider, Pharm.D., or Susan Wheeler, RPh, who have completed a specialized training program in anticoagulation therapy management through the University of Florida.

“Individuals who are taking anticoagulation medication including warfarin or Coumadin® will benefit from our Anticoagulation Clinic,” says Joan Haltom, director of pharmacy services for Ephraim McDowell Health. “We offer point of care testing (INR) during the visits and adjust the dose to reach the targeted INR. Participating in the Anticoagulation Clinic also offers continuity of care for patients to meet National Patient Safety Goals for Anticoagulation Safety.”

During the visit, participants will be educated on the use of anticoagulation medications and will discuss medication and lifestyle changes with the pharmacist. The goal is to help the participant receive the greatest benefit from their anticoagulation medication while also reducing the potential risks associated with it.

A physician must refer and sign a collaborative care agreement for the patient to participate. The benefit for the physician is elimination of time consuming in-office-management of patients on warfarin therapy. Patients must be ambulatory and able to come to the clinic for each appointment. The referring physician will receive a progress report after each visit.
