You may need transitional care as part of your recovery from an illness or injury. If you have been in the hospital longer than three days, you may be eligible. Perhaps you are well enough to leave the hospital but are not able to care for yourself at home. As a result, you may need skilled care services. In addition, you may need the care of a registered nurse around-the-clock. Or you may need treatment that includes extensive rehabilitation.
We offer three transitional care options. Because of them, patients can ease between their hospital stay and being discharged home. Our case managers work closely with you and your family to help select the best option for you. Your payment sources and services you need will determine which option is best for you. As a result, you are matched with the best environment you need for your situation. Because of that, you can fully recover from your illness or injury.
Swing Bed
We offer Swing Beds at Ephraim McDowell Fort Logan Hospital and Ephraim McDowell James B. Haggin Hospital. This is a short-term, temporary placement. It is for patients who require a skilled level of nursing care. Services may include IV therapy, pain management, palliative care, wound care, or physical, occupational or speech therapy. Medicare, Medicare HMO, Veterans Affairs and most private insurance companies cover these services for eligible patients.
Assisted Living, McDowell Place of Danville
McDowell Place of Danville features 98 apartments that offer independent and assisted living. Personal care and respite care services are also offered. Memory care services are also available for individuals living with dementia or other types of memory loss. Residents in assisted living receive assistance with activities of daily living. This includes self-administration of medications. Residents in personal care receive primary care by a nurse including medication administration. Respite care is for short-term stays to recuperate from a recent hospital stay, surgery or fall. Palliative care is also available.
Our residents may contract with an outside home health agency for some services they may need. These include physical, occupational or speech therapy. In addition, they can include IV therapy, skilled treatments for wound care and respiratory therapy. We accept residents who self-pay for their stay. We also accept some long-term care insurances and Veterans Affairs benefits.
